Diabetes is a complex, multisystem disorder that is best managed with a systematic approach. There is evidence that this approach in a primary care setting results in better outcomes and allows GP’s to better manage the burden of diabetes in their practices

Tools available include

  • Electronic Diabetes Register
  • Establishment Of Structured Diabetes Specific Consultations
  • Recall systems
  • Team Managed GP Coordinated Care Treating To Target Using Algorithms Of Care
  • Decision making software
  • Computer tracking systems

These can be implemented and managed as a miniclinic strategy  within the chronic disease model of care

  • The GP, with an interest in the management and care of patients with T2D is appropriately positioned in the diabetes team to offer consistency of care and recall. It is this consistency when allied to the algorithms and GP Total Mini Clinic Model that leads to the improved ABC outcomes that are better than some Diabetes Outpatient Clinics. Using the model and tools developed the general practitioner can algorithmically manage their T2D patients with confidence and success.
  • This model can easily be implemented into any practice.
  • The Mini Clinic approach does not increase workload, but organizes the delivery of care to patients on the register in an efficient treat to target outcome based manner.

Multicomponent organizational interventions can produce significant change in diabetes care and outcomes in community primary care practices (TRANSLATE Trial)

Computer tracking systems have shown :

·         Significant improvement in A1c, FBGL, LDL, TG, BP and a  reduction  in the number of acute visits and hospital admissions
