Meeting the increasing demands for insulin pump therapy and continuous glucose monitoring: a multidisciplinary team learning experience (#358)

Introduction:
Increasing demands for diabetes education services to deliver expertise in continuous subcutaneous insulin infusion (CSII) therapy and continuous glucose monitoring (CGM) created the impetus for a systems change approach for the service. The RMH diabetes multidisciplinary team (MDT) worked together to develop a more coordinated highly skilled approach that focuses on sharing roles and expectations within the team and the patient.
Objectives:
To provide ongoing competency based learning opportunities for members of the MDT and streamline effective processes ensuring consistency for both staff and patients.
Methods:
Conducted MDT strategic planning forum and analysis of existing service data, for example, waiting lists. Instalment of new CSII and CGM software and implementation of hourly training sessions offered to all MDT members over a 9 week period pertaining to CSII and CGM.
Outcomes:
54 MDT staff, including diabetes educators, dietitians, endocrinologists, registrars attended sessions of the program. Earlier referrals for dietetic review and reduction in appointment wait times by 4 weeks identified for CGM and CSII. More effective consultation time spent with patients as a result of improved data upload and interpretation time. The number of face-to-face consultations and follow up appointments were reduced by 2 per person as patients increased the use of telephone and online consultations. A post program evaluation of staff knowledge levels indicates increased skills and confidence in delivery of CSII and CGM.
Limitations:
Exposure to one provider company for the program and time constraints for staff attendance.
Implications for Diabetes Education and Care:
In-house training program for the MDT supporting advanced technological health therapies and diagnostic tools that assists not only with group cohesion, but provides consistent quality care for patients accessing CSII and CGM services. Further work is required to address orientation for new staff and on-going professional development and support in CSII and CGM.
