Auditing the quality of discharge advice given to patients admitted to an Emergency Department (ED) with severe hypoglycaemia (#347)
Introduction
Severe hypoglycaemia can be a life threatening event and major cost burden to healthcare systems (1).
Our literature search demonstrated inadequate patient discharge information to avoid recurrent presentations with hypoglycaemia (2, 3, 4). Standardized information for patients in ED has not been documented. The DAWN study reported hypoglycaemia as the “most feared” diabetes complication by patients. (4)
This project was accepted as a QA activity by the Research Governance Unit at our hospital.
Aims:
1. Audit hypoglycaemia attendances in ED
2. Ensure ED consultants have access to evidence based advice, when discharging patients who have presented to ED with hypoglycaemia.
Objectives:
1. Identify patients presenting to ED with hypoglycaemia, during a retrospective six month period.
2. Document discharge advice and education provided to patients.
3. Establish a working group to develop an “ED Hypo Information Package”
4. Develop and roll out staff education re “ED Hypo Information Package”
5. Evaluate the usefulness of the “ED Hypo Information Package”
Methodology
Retrospective data were collected over a 6 month period
Results
The preliminary data includes a review of 19 ED admissions for hypoglycaemia
Evaluation:
• Preliminary results are consistent with the literature review findings, in that only a minority of patients had documented evidence that advice was given on how to prevent recurrent hypoglycaemia.
• Planned audit post six month roll out to compare data.
• Staff will be surveyed on usefulness and ease of access.
